Position Description:
The Client Care Coordinator works to enhance the effectiveness in achieving our agency's goals of the high-quality care and customer service while demonstrating compliance with all regulatory agency standards. The Client Care Coordinator works directly with the client, the client’s family, and the care team. Each clients’ care team consists of the Scheduling team, the Caregivers, and any 3rd party directly involved with the care of the client.
Position Responsibilities:
· Collaborates with the care team regarding a client’s plan of care.
· Serve as an educator and resource for each client (and family) in managing their own care in order to ensure optimal quality of care.
· Identifies, evaluates, and acts to resolve any potential barriers to delivery of care.
· Visit each client on a monthly basis or as needed.
· Arrange for additional care appropriately according to needs of client.
· Participate in the educational and training process of caregivers at orientation, in the field and one on one in a controlled environment.
· Inform scheduling of client’s needs and assist with assigning a caregiver.
· Communicate with scheduling before signing up a new client concerning schedule availability.
· Follow up with scheduling daily to ensure accountability.
· Work with Office Manager and Medicaid Specialist on authorization authenticity.
· Put schedules in the system according to the authorization.
· Ensure paperwork is compliant with state guidelines.
· Include HR Coordinator in all caregiver issues.
· Bring all compliance concerns to the office manager in a timely manner.
· Other duties as assigned.
Position Requirements:
· One year of Case Management or two years of Geriatric care experience.
· Working knowledge of the components of a client’s care needs specifically related to the area/function in which care management will be performed.
· Excellent analytical and interpersonal communication skills necessary to negotiate with families, clients, Physicians, and third-party payers for optimum care.
· Demonstrated ability to work well with other professionals in a direct and positive manner.
· Demonstrated ability to operate all equipment necessary to perform the job (e.g. FAX, computer, copier, telephone system, etc.)
· Ability to pass an AHCA Level II Background check.

Pay: From $45,000.00 per year
