Position Summary: The HCBA Waiver RN Supervisor has the responsibility for providing direct and indirect nursing care to the HCBA Waiver participant as well as administration and supervision of the HCBA WAIVER nursing team. Works under the supervision of the HCBA Waiver Manager but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which they are responsible. Essential Functions of the Job: Monitor the day-to-day RN activities in the HCBA Waiver Program. Develops and implements appropriate/required HCBA Waiver nursing protocols and monitors that nursing team is following those protocols. In coordination with the HCBA Waiver Manager, HCBA Waiver Social Worker Supervisor, and Program Supervisor, ensures that Program protocols, policies, and DHCS' guidelines are followed, reviewed, and updated as needed and regular audits are conducted; demonstrates active participation in HCBA Waiver QA processes; is involved in assessment and implementation of QA improvement plans. Conducts regular audits to ensure the HCBA Waiver clinical area is meeting HCBA Waiver requirements and regulations. Assists with the planning and preparation of the HCBA Waiver audits. Participates as a member of the Interdisciplinary Team; attends staff meetings and takes part in participant care planning. Leads and coordinates Nursing Team Meetings, training of nursing team, and on-going review of policies and procedures. Responsible for assessing and implementing nursing team competencies to all newly hired nursing team members. Conducts and reviews HCBA Waiver participant assessments as needed. Initial and Periodic assessments (minimally every 6 months) to be completed prior to the scheduled team meeting. Ensures the development of a plan of care based on nursing process and incorporates the plans of other disciplines and continuing care needs; includes the patient/family in developing or revising the plan of care; makes referrals to multidisciplinary support services. Communicates participant changes to team members; coordinates participant care as needed. Conducts homes, hospitals, Skilled Nursing Facility, etc. visits and assessments as requested/needed. Performs advanced nursing assessment using critical thinking skills. Ensures the care to be provided conforms to accepted practice standards. Ensures participant and family education is provided by the nursing team as needed. Is responsible for coordination of care delivery including coordination. Identifies emergency situations and independently initiates proper protocol. Acts as coordinator of Participant's care with other health care personnel; coordination of care and discharge planning with hospitals and skilled nursing facilities; understands and demonstrates respect for Participant's rights and utilizes established mechanisms for management of ethical issues in Participant's care. Records Participant's care delivered as planned and any variation, with appropriate rationale; makes and records observations related to impending or associated problems. Is responsible for overall performance and timesheet management for HCBA Waiver Nursing Team; integrates cost effective measures into nursing practice. Recognizes direct team problems/issues and takes responsibility for documenting same. Complies with policies addressing safe working conditions; monitors unsafe working conditions; recognizes inappropriate and/or ineffective Participant's care management, resolves issue/problem, and completes written reports; fulfills call for unit as assigned and adjusts staffing for census (volume) and acuity. Job Requirements Experience Required: Has practiced nursing in the last year. 2 years RN experience with at least 9 months experience working with HCBA Waiver or any other program providing coordination of care. Preferred Experience: Knowledge and/or experience working in a HCBA Waiver program, Nursing Home, Hospice, ADHC. Nursing competency and familiarity with care paths, interactive tools commonly used in skilled nursing home settings. Must be able to triage clinical symptoms, apply care paths and clinical interactive tools in Participant evaluation and care prior to communications with primary care providers. Must be comfortable with acute frail patients, and clinical and social complex cases. Education Required: Graduate of an accredited school of nursing. Certification Required: Currently licensed in CA as a Registered Nurse license; must present credentials which demonstrate ability to meet performance criteria for RN. BLS Certification Verbal and Written Skills Required to Perform the Job: Ability to read, write and comprehend medical information and terminology. Ability to effectively present information, both verbal and written. Ability to effectively document in EMR (efficient typing, thorough progress notes and navigation of EMR) Good knowledge of Microsoft Applications including Word, Excel, Access and Power Point. Computer data entry, analysis and reporting experience required. Technical Kowledge and Skills Required to Perform the Job: Must possess the skills to provide care for complex patients, Must thrive in team environment, possess good organizational and supervisory skills, ability to effectively handle difficult and unusual interpersonal situations. Knowledge of Electronic Health Data Systems. Knowledge of managed care regulations (state and federal). Principles and practices of health care service delivery, managed care, health care systems, and medical administration. Experience performing audits analyzing productivity and quality of utilization management Knowledge and/or experience with the senior care market, including competitors, regulations, and available resources. Experience in the application of InterQual criteria or other evidence based medical criteria. Meet a standardized set of competencies for the specific position description established by San Ysidro Health HCBA Waiver and approved by the State of California. Equipment Used: Minor medical equipment such as vital signs monitors, emergency cart, automated external defibrillator (AED), and Participant personal assist devices. Networked computer with various applications, facsimile machine, document center, and telephone. Working Conditions and Physical Requirements: Physical clearance for reaching, bending, stooping, crouching, kneeling and grasping. Have full range of motion. Ability to lift 50 lbs. T.B. clearance. Must maintain current First Aid and/or CPR. Frequent standing and walking indoors and outdoors. Will be required to travel to other sites or other locations in San Diego County and Imperial County. Universal Requirements Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs. Compliance with all mandated vaccinations and all boosters is a term and condition of employment.
