POSITION SUMMARY

Under general supervision, this position functions as a direct link between families enrolling in CAPC’s trauma-informed, culturally responsive Family Services program and the agency in accordance with the funding contract. The program aims to reduce youth involvement in firearm-related violence, strengthen family protective factors, and promote social-emotional development through mentoring, case management, family support, emotional regulation, violence prevention, and school re-engagement services.

The Family Advocate is responsible for outreach, intake, and enrollment of families, as well as comprehensive assessments of families’ strengths, needs, and protective factors. This role provides parent coaching, case management, and connection to community resources to improve family functioning, resilience, and long-term well-being.

The Family Advocate works in close collaboration with the Youth Advocate, ensuring coordinated, trauma-informed, and culturally responsive support for caregivers. Together, they foster trust, strengthen protective factors, and guide families in building skills, navigating challenges, and developing connections that reduce risk and promote positive outcomes for families.

PROGRAM SPECIFIC DUTIES:

The duties listed below are intended to describe the essential functions of the position and are not intended to be an exhaustive list of all responsibilities.

  • Outreach & Engagement
    • Conduct proactive outreach to eligible families in alignment with funding contract requirements and program eligibility criteria.
    • Establish trusting, culturally responsive relationships with caregivers using trauma-informed engagement strategies.
    • Present program services clearly to families, ensuring understanding of voluntary participation and program goals related to violence prevention and youth development.
  • Intake & Enrollment
    • Complete intake screenings, eligibility verification, and enrollment documentation in compliance with contract and agency standards.
    • Maintain accurate and timely documentation in required data systems and case records.
  • Comprehensive Assessment
    • Conduct strengths-based, trauma-informed assessments/screenings to identify family needs, protective factors, risk factors, and service priorities.
    • Assess domains including family functioning, social-emotional development, school engagement, community safety, and access to basic needs.
    • Collaboratively develop individualized family service plans that align with program goals of reducing firearm-related violence and strengthening protective factors.
  • Parent Coaching & Skill Development
    • Provide individualized parent coaching focused on emotional regulation, positive discipline strategies, communication skills, and strengthening caregiver-child relationships.
    • Deliver structured, curriculum with families, guiding caregivers through lessons on trauma, communication skills, positive parenting, and family strengthening. Ensure fidelity to the curriculum and integrate lessons into individualized service plans.
    • Support caregivers in developing protective factors such as resilience, social connections, knowledge of parenting and child development, and access to concrete supports.
    • Model and reinforce trauma-informed and culturally responsive parenting approaches.
  • Case Management & Resource Navigation
    • Provide ongoing case management services, including goal tracking, follow-up, and reassessment of needs.
    • Connect families to community-based services such as mental health, substance use treatment, housing, employment, food assistance, and educational resources.
    • Coordinate referrals and warm hand-offs to ensure successful linkage to services.

6. Violence Prevention & Safety Planning

  • Support caregivers in identifying and addressing risk factors related to youth involvement in firearm-related violence.
  • Collaborate with families to develop safety plans and prevention strategies that promote safe environments at home, school, and in the community.
  • Reinforce strategies that promote nonviolent conflict resolution and emotional regulation within the family system.

7. School & Youth Support Collaboration

  • Partner with the Youth Advocate to ensure coordinated service delivery and shared case planning.
  • Support caregiver engagement in youth mentoring, school re-engagement efforts, and academic advocacy.
  • Participate in joint case reviews and multidisciplinary collaboration as appropriate.

8. Data Collection & Reporting

  • Collect required program data, outcome measures, and service documentation in compliance with funding requirements.
  • Monitor progress toward measurable outcomes related to family protective factors, youth engagement, and violence prevention.
  • Participate in program evaluation activities and quality improvement initiatives.

9. Community & Interagency Collaboration

  • Develop and maintain relationships with community partners, schools, health providers, and violence prevention networks.
  • Represent the program in meetings, case conferences, and community collaborations as assigned.
  • Advocate for family needs within systems of care while maintaining confidentiality and professional boundaries.

10. Professional Practice & Compliance

  • Adhere to agency policies, confidentiality standards, and mandatory reporting laws.
  • Participate in ongoing professional development related to trauma-informed care, cultural responsiveness, violence prevention, and family engagement.
  • Support program sustainability by ensuring compliance with contractual and grant-funded expectations.

MINIMUM QUALIFICATIONS:

  • Education:
    • Graduation from an accredited four-year college or university with a major in social services, child development, or a closely related field is preferred.
  • Experience:
    • Minimum of two years of relevant experience working with children, youth, and families in supportive, case management, or community-based roles.
    • Demonstrated ability and enthusiasm for engaging and supporting families, particularly parents and caregivers, in building strengths, resilience, and protective factors.
    • Experience in trauma-informed, culturally responsive, or community-based programs is highly desirable.
    • Lived experience and strong connections within the community are considered valuable assets.
  • Knowledge of:
    • Child welfare issues, childhood development, and child abuse prevention.
    • Community-based social networks and resources relevant to families and youth, including knowledge of the Stockton area and its local organizations.
  • Technical Skills:
    • Intermediate knowledge of Microsoft Office - Word, Excel, Access, PowerPoint, Publisher, Outlook.
    • Ability to learn server general maintenance functions and other software as required.
  • Other Requirements:
    • Bilingual English/Spanish skills are required due to the needs of the population served.
    • Ability to work flexible hours, including occasional evenings or weekends, for family engagement and community events.
    • Strong interpersonal skills, including cultural competence, motivational interviewing, and relationship-building abilities with youth and families

CONDITIONS OF EMPLOYMENT:

  • Must pass a health physical and provide a T.B. clearance.
  • Employment is contingent upon successful completion of Department of Justice - DOJ - fingerprint clearance in accordance with applicable law. CAPC complies with California’s Fair Chance Act regarding the consideration of criminal history.
  • Must agree to a DMV record report compliant with the agency’s liability insurance requirements.
  • Must have a car, a valid California driver’s license, and proof of automobile insurance. If driving on agency business, the employee must maintain insurance coverage that meets the minimum requirements of California law and agency policy.
