Introduction

Under the direction of the Behavioral Health Director, the Behavioral Health Medical Director administers the medical services of San Joaquin County's Behavioral Health Services (BHS) and is responsible for the direction and management of its psychiatric and medical staff operations.

The BHS Medical Director will develop, interpret and enforce policies, procedures and processes regarding the managed care system of providing psychiatric medical services. The BHS Medical Director will select, supervise, assign, train and evaluate all psychiatric and medical staff of BHS. The BHS Medical Director will monitor the quality of care for mental health and substance use disorder clients, including those with co-occurring disorders, in multiple treatment sites for all clients, including children, adults and older adults.

The BHS Medical Director will coordinate with medical staff from other departments within the Health Care Services Agency, including San Joaquin General Hospital and Public Health Services. The BHS Medical Director will also analyze regulations and legislation for impacts on mental health and substance use disorder services and implement appropriate compliance procedures.

TYPICAL DUTIES

The BHS Medical Director must maintain all medical licenses to practice medicine by the Medical Board of California and Drug Enforcement Administration (DEA) as well as required CMEs in addiction medicine. The BHS Medical Director must adhere to and follow all Federal and State regulations and guidelines for the provision of and billing reimbursement on behalf of the County by Medi-Cal/Early Periodic Screening, Diagnosis, and Treatments (EPDST)/Medicaid, Medi-Care, Healthy Connections, Federally Qualified Health Centers (FQHC), Drug Medi-Cal and Private Insurance as appropriate, and will participate in Peer Reviews of medical practice and records, as assigned. The BHS Medical Director will check and respond to voice mail and e-mails daily (Monday through Friday) to ensure optimal communication and will process all grievances and change of provider requests within regulatory timelines.

BHS Compliance Plan and Documentation Procedures must be followed, including but not limited to: progress notes, and completion of all documentation per regulations. In addition, all Health Insurance Portability and Accountability Act (HIPAA) regulations will be followed, including any and all County Security and Privacy Policies. BHS psychiatrists will comply with Exhibit B for Medi-Cal providers. The Medical Director will serve on the credentialling committee as a member. The Medical Director will ensure compliance with timeliness standards and work to move the agency towards improved HEDIS measures.

The BHS Medical Director will follow the Medi-Cal Medication Formulary and Black Box warning procedures, adhering to Community Standards of Practice for the prescribing of psychotropic medications and will adhere to the use of BHS Formulary and Department of Health Care Services Treatment Authorization Request Procedures. To ensure that BHS is reimbursed for paid services, the Medical Director will be responsible for overseeing the psychiatric medical staff in maximizing billing potential. All services will be documented using the guidelines outlined in the Medi-Cal billing manuals.

BHS is responsible for the psychiatric services at the county jail. The Medical Director will ensure inmates of the county jail receive timely and appropriate psychiatric services. Correctional Health Services (CHS) at the county jail rely heavily on Nurse Practitioners, including Psychiatric Mental Health Nurse Practitioners (PMHNP). The Medical Director will ensure the PMHNP’s assigned to the county jail and other BHS programs have appropriate onboarding and oversight.

There is an expectation that the BHS Medical Director will continue to provide direct services to patients. The percentage of time dedicated to direct service will be dictated by agency need. The BHS Medical Director is the provider of last resort and will see patients with challenging presentations as appropriate and necessary.

MINIMUM QUALIFICATIONS

The BHS Medical Director is required to possess and maintain all medical licenses to practice medicine by the Medical Board of California and Drug Enforcement Administration (DEA) as well as required CMEs in addiction medicine.

Board Certification in Psychiatry is strongly preferred.

Board Certification or training in Addiction Medicine and/or Child Psychiatry is desirable.
