Role Summary:
The Patient Care Coordinator ensures patients’ electronic medical records are up-to-date and accurate; provides timely follow-up and completion of referrals made by providers; responds to all external requests for medical records; and provides general administrative support for all aspects of clinic operations. This position reports to the Clinic Manager.
Key Responsibilities:
- Ensures timely completion and follow up of medical specialty referrals to external facilities.
- In coordination with case management team, assists with patient appointment scheduling for specialty referrals, as needed.
- Processes and completes durable medical equipment referrals placed by providers.
- Manages workflows pertaining to patient form (i.e. IHSS, SDI, para transit, etc) completion for patients.
- Scans and files clinical, financial and administrative documents into patients’ electronic health records.
- Responds to requests for patient medical records and forwards information in a timely manner, as mandated by California law.
- Reviews and sorts faxed documents and forwards to the appropriate provider and patient chart in a timely manner.
- Complies with HIPAA rules and regulations at all times.
- Answers and responds to requests from both internal and external customers.
- Forwards outside providers' requests for urgent health information to appropriate provider when a signed release of information is not available, in compliance with HIPAA privacy rules for disclosure when patient cannot consent.
- Prepares, scans, files and abstracts historic patient information into electronic health record.
- Prepares invoices for medical suppliers and vendors.
- Performs other duties as assigned.
Qualifications:
- High school diploma or equivalent
- 1 year of experience of working with patient medical records and managing medical referrals
- Ability to use an electronic health record (e.g. OCHIN Epic)
- Excellent attention to detail
- Strong interpersonal skills
- Self-motivated, independent learner and problem solver
- Excellent computer skill
- Must obtain TB clearance (within past 12 months) and complete background check upon hire.
- Current with vaccinations including latest Covid-19 vaccine with at least one booster, and seasonal flu, and able to provide proof.
Preferred:
- Bilingual Spanish/English preferred
- Previous clerical or office experience preferred
- Familiarity with medical terminology and ICD-10 and CPT codes preferred
- Willingness to occasionally travel between SFCHC sites – 730 Polk Street and 1800 Market Street
- Experience working in an FQHC or other type of Community Health Center.
