Program Descriptions:
The REACT Program is a 16-week program that aims to provide modules regarding medical, behavioral health, and substance use. REACT provides case management for clients to connect to medical, behavioral health, dental, harm reduction, and most importantly substance use treatment. Targeted populations include mainly people who use substances, people experiencing homelessness, LGBTQ community members with an emphasis on transgender individuals, and the black/African-American and Latinx communities. The REACT program hopes to transition clients after program to peer-led groups and continued care through SFCHC.
The RESTORE Program aims to connect residents to medically-assisted substance use treatment, medical care, trauma-informed mental health care, social services, housing and shelter services, and referral services during their temporary stay at the Adante Hotel. Residents are primarily persons experiencing homelessness, persons experiencing mental health issues, substance abuse, or other crises; and, all are Tenderloin residents outreached during alternative hours by the Night Navigator Program: an essential engagement and service gap in the neighborhood by providing services to the hardly reached individuals in need during alternative operating hours. A collaboration with San Francisco Department of Public Health (SFDPH) and Code Tenderloin (Code TL), the RESTORE Program aims to move participants towards stabilization within their community by supporting them on their journey towards more permanent housing and through harm reduction and substance use reduction/treatment.
Position Summary:
The Case Manager is responsible for providing assessments, including onboarding, offboarding, and follow-up assessments, and linkage to the necessary resources to seamlessly move clients through the REACT and RESTORE programs. The Case Manager collaborates and works with other Case Managers, Community Health Workers, and Community Ambassadors within SFCHC programming and other organizations, such as SFDPH and Code TL, to ensure case management services are provided during and after program. The Case Manager is also responsible for working with their assigned case load to develop care plans and coordinating participants through navigating medical care, health education, supportive counseling, benefits advocacy, and coordinating services within their program as assigned by their direct supervisor, the Program Manager of Community Engagement. Direct client work and interaction will be done both in the field such as the Adante Hotel and at the office at our main clinic at 730 Polk Street, or at our various satellite locations within San Francisco.
Essential Duties:
- Conduct intakes and assessments of new clients for these programs and other SFCHC services, including offboarding assessments, and post-program follow-up assessments.
- Provide supportive counseling and develop service goals with clients.
- Provide information and referrals to external services and monitor referral completion.
- Work with the client’s care team to identify their needs and develop goals accordingly.
- Navigate and outreach clients as needed to support with care engagement.
- Provide advocacy as needed, general health education, and ongoing referral and linkage to health services.
- Participate in community events to identify potential program participants.
- Complete documentation, reporting, and treatment plans in a timely manner.
- Facilitate and support educational workshops and groups.
- Work collaboratively with staff within the Community Engagement Department, SFDPH, and partnering organizations, through supporting clients accessing programming.
- Work collaboratively with SFCHC staff and assist as-needed with field-based outreach and program promotion activities.
- Attend staff meetings and trainings as required.
- Participate in, and comply with, agency quality assurance, peer review, and clinical consultation.
- Support the Community Engagement department with all aspects of the Community Living Room, a daily drop-in services for community members to access food and linkage to necessary services such as harm reduction, programming, and support groups.
- Perform other duties and tasks as needed.
Minimum Qualifications:
- One year of experience providing case management, counseling, advocacy, health education, service coordination, and/or outreach work to disenfranchised communities.
- Experience working with diverse populations including BIPOC, the LGBTQ community, refugees/immigrants, people living with HIV/AIDS, people experiencing homelessness, and multiple diagnosed populations (substance use and/or behavioral health issues).
- Knowledge of harm reduction philosophy, client centered counseling, and motivational interviewing techniques and trauma informed care.
- Must be adaptive, flexible and a team player.
- Strong verbal and written proficiency in English.
- Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills.
- Familiarity and experience with Microsoft Word and Outlook programs
- Detail-oriented with strong follow-through skills, including attention to detail.
- Must be able to sit, stand, and walk for extended periods of time.
- Fully vaccinated against COVID-19, complete with booster shot, and able to show proof of vaccination.
- Up to date with vaccinations, including seasonal flu shot vaccine.
- Proof of TB clearance (issued within the past 12 months).
Preferred Qualifications:
- BA/BS in a health sciences or social work-related field or AAA/CMH/CHW OR comparable work experience.
- Competency in Spanish language (highly preferred).
