Overview
A comprehensive perinatal health worker (CPHW) provides patient support, education, and case management under California's Comprehensive Perinatal Services Program. The CPHW conducts patient assessments, creates individualized care plans, tracks patient progress in Electronic Medical Records (EMR), provides nutrition counseling, screens for psychosocial issues like domestic violence, connects patients with community resources, and ensures proper documentation and reporting.
Responsibilities
- Conduct prenatal and postpartum patient assessments, including medical histories and psychosocial factors.
- Develop individualized care plans for patients in the prenatal program.
- Track patient progress, including weight and other health outcomes.
- Educate patients on pregnancy, labor, delivery, postpartum care, nutrition, and healthy lifestyle practices.
- Provide counseling and guidance on managing discomfort and avoiding harmful practices.
- Explain CPSP and Medi-Cal services and other available community support.
- Assess patients' needs and provide information about community resources and referrals.
- Assist patients in navigating the healthcare system and accessing necessary services.
- Utilize the clinic's Electronic Medical Record (EMR) system to document patient information and appointments.
- Complete and submit reports and data for the CPSP program.
- Assist with patient checkout processes, such as scheduling follow-up appointments and processing referrals.
- Coordinate with Medical Assistants and other clinic staff to ensure comprehensive patient care.
- Actively participate in clinic meetings, trainings, and other professional development activities.
- Act and assist providers in capacity of a medical assistant
Qualifications and experience
- California CPSP certification.
- Medical Assistant certificate.
- High school diploma.
- Ability to work effectively both independently and as part of a team.
- The role requires strong communication skills, cultural sensitivity, adherence to clinic policies and CPSP protocols, and the ability to work collaboratively with a clinical team under the supervision of a physician.
- Bilingual English-Spanish required.
Job Type: Full-time
Pay: $24.00 - $30.00 per hour
