Hospice RN Case Manager Responsibilities
$50.00-$60.00 per hour

Hospice RN Case Manager Responsibilities
- Perform RN duties on hospice team.
- Serve as a RN Case Manager for the hospice Interdisciplinary Team
- Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
- Establishes plan of care while collaborating and communicating with the Interdisciplinary Team including physician when appropriate
- Coordinates all hospice services provided to patients/families in all settings to ensure quality of care
- Registered Nurse with valid CA state license
- Minimum one year of hospice RN experience
- Strong background in medical/surgical, ICU/CCU, or ER is helpful
- Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patients place of residence
- Must have excellent interpersonal skills and a passion for hospice
- Understanding of hospice philosophy and needs of the terminally ill
- Self-directed with ability to work with little supervision
- Current Physical and TB test results
