As a Home health RN, you will develop a plan of care in collaboration with the patient, family/caregiver, and physician orders including frequency and duration of nursing visits, specific care or treatments to be provided, appropriate goals, medications, and other disciplines needed. You will make regular visits to monitor the patient's conditions, illnesses, or injuries.RESPONSIBILITIES:Perform comprehensive assessments in the patients' homes and identifies patient's needs.Develop a care plan in collaboration with patient/CGs, healthcare provider, case manager, and/or supervisor as assigned and document appropriately.Assess the need for additional services and equipment on admission on an ongoing basis and inform the case manager and/or supervisor on a timely basis.Assess the home environment and make recommendations that will enhance patient care, promote safety, and prevent adverse effects.Take verbal orders from the healthcare provider as necessary promptly and documents appropriately.Initiate appropriate preventative, rehabilitative, therapeutic, comfort, and pain interventions and provide instructions to patients/CGs and staff promptly.Demonstrate professionalism by maintaining high standards of clinical care and staying current on clinical and medication issues.Updates to patients' clinical records including medication profiles, plan of care, OASIS, case conference, progress notes, and re-certification documentation as required.Serve as a preceptor when neededOther duties as assignedWHO WE'RE LOOKING FOR:You are experienced and ready to bring your skills and knowledge to patients who need it most.
