Part Time (approximately 3 days per week) Occupational Therapist (OT)POSITION SUMMARY:As a Home Health Occupational Therapist, you will evaluate and treat patients with injuries, illnesses, or disabilities in their homes. You will help patients recover, improve, and maintain skills needed for daily living.RESPONSIBILITIES:Evaluate patient functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.Evaluate home environment for hazards or barriers to more independent livingIdentify equipment needsProvide supervision to the Occupational Therapy AssistantsConduct an initial evaluation/assessment and report findings to the Health Care Provider and RN and develop a plan of care promptly for patientsDocument initial evaluation and treatment plan and re-evaluations of patient’s progress at least every thirty (30) days and update plan of care as necessary. Complete comprehensive assessments and OASIS data, as indicated on a timely basis.Observe, record, and report to the Health Care Provider the patient’s treatment, response, and progress and promptly report significant changesObtain orders to revise the plan of careInstruct patient/CG in the use of alternate methods and devices for upgrading independence in activities of daily livingWHO WE’RE LOOKING FOR:You are passionate about restoring function, relieving pain, and preventing disability by engaging with patients in the most meaningful setting – the home.
