- Develop effective treatment plans and obtain necessary approvals from referring physicians.
- Provide patient care according to the physician-approved treatment plan.
- Communicate regularly with supervisors and other healthcare team members regarding patient progress, challenges, and plans.
- Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
- Engage in in-service training programs for facility staff.
- Maintain accurate documentation of evaluations, daily treatment notes, weekly progress reports, and recertification documentation in accordance with the Speech and Language Pathology Board State Practice Act and regulatory requirements.
- Document treatment changes per facility policies and procedures.
- Educate patients' families or nursing staff on maintenance programs and provide caregiver training in preparation for discharge.
- Participate in discharge planning to ensure a smooth transition for patients.
- Serve as a clinical preceptor for affiliating CFY/RFP Speech-Language Pathologists and student programs.
- Comply with the Speech-Language Pathology and Audiology Bureau State Practice Act and facility policies.
- Build positive, effective relationships with patients, residents, families, staff, and professional colleagues.
