What are the responsibilities and job description for the Wound Care Nurse Navigator position at San Antonio Regional Hospital?
Overview
POSITION SUMMARY
Functions as an expert clinician in Wound and Enterostomal care, and assists patients through the continuum of care and treatment regarding Wound and Enterostomal Care. Provides care and education to patients and their families with select disorders of the gastrointestinal, genitourinary, and integumentary systems. Collaborates with all members of the interdisciplinary team to anticipate and respond to changes in patient health status and patient/family perceptions of care.
MINIMUM QUALIFICATIONS
Education: Bachelor’s Degree in Nursing (BSN) or related field required.
Experience: Three years acute care experience. Previous experience with program development and implementation of specialized would care treatment. Previous experience with policy development, strong knowledge of CDPH regulatory requirements regarding documentation and wound care programs.
Knowledge and Skills: Able to work in a fast-paced environment; and manage/prioritize/complete work assignments and other duties, as assigned.
License and Certifications: Current RN license, state of California. Current American Heart Association (AHA) BLS card. Graduate from a CWOCN (certified wound, ostomy, continence nurse) accredited enterostomal therapy nursing education program within twelve months of hire.

Physical Requirements: Able to complete the essential physical duties as noted on the job desciption.
PAY RANGE
$49.10 - $73.64
The posted pay range reflects the lowest to highest pay that was available for this position at the time of posting and may be subject to change. Salary offers are determined by candidate’s relevant experience and skills. For per diem positions, a standard rate is used based on market data and not the candidate’s individual experience.
