Job Description
Title: Mental Health Counselor (MSW or MFT)
Reports to: Licensed Clinical Social Worker (LCSW)
FLSA: Full Time Exempt
Position Summary:
This position is under the supervision of the Licensed Clinical Social Worker (LCSW), with the responsibility of providing therapy for patients and coordinating care with their primary care provider (PCP). Their job also includes working with socially excluded people. They maintain a good professional relationship with other people wherein they act as an advocate and guide them towards recovery.
Qualifications:
Masters degree in Social Work or equivalent from an accredited program essential;
Must be able to show sensitivity to the mental health needs of an underserved community;
Must be committed to patient and community education;
Must have strong clinical skills and the ability to develop quality treatment planning;
Must be committed to the mission of SDFHC, Inc;
Current CPR/BCLS certificate;
Working computer skills including MS Office, EMR and other software systems; and
Bilingual, English and Spanish, preferred.
Experience:
Minimum of one year experience in the performance of the below described duties under the guidance and supervision of an LCSW or equivalent.
Additional Requirements:
Ability to pass an initial and annual physical assessment and TB test and pass an initial background check conducted by the agency. Must be able to present a positive image of the organization to members of the community and have effective communication skills, both oral and written, and strong organizational, problem solving and time management skills. Must be able to observe and record information, including non-verbal information, and learn and apply agency policies and procedures as well as any regulatory standards. Must be familiar with Electronic Health Records and use of electronic equipment such as computers, fax machines, scanners and telephonic devices.
Working Conditions:
Due to the nature of Community Health Centers, this job may involve infrequent exposure to blood, bodily fluids or tissues and possible exposure to communicable diseases. This job involves sitting for long periods of time as well as stooping or squatting and must be able to lift 20 pounds.
Schedule:
Generally a Monday – Friday 8a-430p. However, the schedule may change based on the need of the patients served and the organization.
The qualifications listed above are guidelines for selection purposes; alternative qualifications may be substituted if sufficient to perform the duties.
Essential Functions (includes but not limited to):
1. Conducting bio-psychosocial assessments and social histories;
2. Assessing clients for substance use, support systems, physical and emotional functioning, financial stability, safety, suicidal/homicidal ideation, etc;
3. Providing direct therapeutic services such as individual, family and group therapy regarding a specific mental health issue;
4. Assist families and individuals experiencing various personal issues such as mental disabilities, physical disabilities, alcohol or drug addiction, extreme poverty and domestic abuse;
5. Responsible in making certain about patient’s safety as well as encourage patient to verbalize their feelings and provide other therapeutic activities that allows them to express their suppressed emotions;
6. Providing crisis management including assessment for safety;
7. Developing and implementing treatment plans and discharge plans;
8. Achieve quality, complete, and timely clinical documentation;
9. Advocating for client services;
10. Providing case management services including, but not limited to, referrals to community resources, collaboration with other professionals, etc.;
11. Promoting mental health services provided by the health center to the greater community;
12. Participating in grant writing and evaluation to garner or maintain program funding;
13. Identifying and resolving ethical issues;
14. Provide in-service training as needed;
15. Functioning as part of a multidisciplinary treatment team; and
16. Performs other duties, as requested.

Job Type: Full-time

Pay: $50,599.00 - $62,812.00 per year
