The Sampson County Health Department seeks a motivated and energetic team player to assist with promoting and protecting the public health through enforcement of state and local environmental laws, educational programs, and consultations with the public.
DESCRIPTION OF WORK: This position is responsible for an assigned patient case load of County Residents for both Pregnant Women and High-Risk Children Programs. Each patient assignment is made of receipt of a referral from a medical provider or other agencies providing prenatal, postpartum, and newborn services to County Residents. Responsibilities include assessments, evaluations, developing patient care plans using independent judgement, patient monitoring, communication with providers and assisting patients and families with utilizing community resources. This position does require home visits.
MINIMUM EDUCATION AND EXPERIENCE: Graduation from an accredited school of professional nursing and one year of professional nursing experience. Must hold and maintain a current license to practice registered nursing in the state of North Carolina by the N. C. Board of Nursing.
