Job Overview
We are seeking a compassionate and highly skilled Hospice Nurse to join our dedicated healthcare team. The ideal candidate will provide exceptional end-of-life care, focusing on pain management, symptom control, and emotional support for patients and their families. This role requires a strong background in various nursing disciplines, including critical care, palliative medicine, geriatrics, and home & community care. The Hospice Nurse will work closely with interdisciplinary teams to ensure patients receive comprehensive, personalized care in accordance with healthcare standards and regulations. Experience with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks is preferred. Responsibilities
- Deliver comprehensive patient assessments to determine needs related to pain management, symptom control, and emotional support.
- Administer medications and injections accurately while adhering to aseptic techniques and HIPAA regulations.
- Manage complex medical equipment including ventilators, IV infusion devices, catheters, and wound care supplies such as ostomy care.
- Coordinate discharge planning and collaborate with interdisciplinary teams for seamless patient transitions across settings such as hospitals, nursing homes, assisted living facilities, or home care environments.
- Provide education and coaching to patients and families on disease processes like Alzheimer's disease, dementia care, or chronic conditions such as infectious disease care or trauma medicine.
- Document all patient interactions thoroughly within electronic health records systems like Epic or Cerner.
- Monitor vital signs, conduct physical examinations, and perform diagnostic evaluations including laboratory specimen collection/processing and medical imaging when necessary.
- Maintain infection control standards and ensure sterile processing protocols are followed during procedures like sterile dressing changes or catheter care.
- Participate in ongoing training related to hospice & palliative medicine, pain management techniques, ICD coding (ICD-10/ICD-9), CPT coding, and other relevant medical documentation practices.
- Respond effectively to urgent situations such as airway management or acute pain episodes within the scope of hospice care.
Experience
- Proven experience in hospice & palliative medicine is essential; familiarity with post-acute care settings is highly desirable.
- Background working in diverse environments such as hospitals (including Level I Trauma Centers), ICU/CCU/PICU units, emergency rooms, outpatient clinics, or home & community care is preferred.
- Experience administering injections, IV insertion/infusion therapy (including dialysis), tube feeding management, and ventilator support is required.
- Prior work with vulnerable populations such as children (pediatric experience), elderly (geriatrics), or individuals with disabilities enhances suitability for this role.
- Knowledge of medical terminology related to trauma medicine, infectious disease control, neurodegenerative disorders like Alzheimer’s/dementia care is advantageous.
- Familiarity with EMR/EHR systems such as Epic or Cerner along with proficiency in ICD coding (ICD-10/ICD-9) ensures efficient documentation practices.
