What are the responsibilities and job description for the Social Services Staff position at Salyersville Nursing and Rehabilitation Center?
Position Purpose:
Assist the social services Director in identifying and providing for resident's social, emotional and psuchological needs, and the continuing development of the resident's full potential.
Essential Funtions of the Position:
- Assist in collection social history data and in the development of a comprehensive dishcarge plan, including the MDS and care plan for new admits, as directed by the social services director.
- Orient and assist new residents in adjusting to the facility. Assist residents and families with adjustment to changes, including room changes.
- Assist families with engraving and/or marking of resident's personal items.
- Assist in facilitating family group meetings.
- Document progress notes, which relate to each resident's care plan when necessary and within policy timeframe.
- Participates in departmental budget and supplies monthly report to Administrator.
- Perform addtional duties as assigned.
Education/Requirements:
Bachelores Degree Preferred plus a minimum of 1 year experience in social work, preferably in a long term care facility; or equivalent combination of education and experience. Highest level of professionalism with the ability to maintain confidentiality. Ability to communicate at all levels of organization and work well within a team enviroment in support of comapny objectives.
