Job Summary
We are seeking a Registered Nurse (RN) for a contract opportunity in Mount Clemens, Michigan. This role involves providing direct patient care in a long-term acute care hospital (LTAC) while collaborating with interdisciplinary healthcare teams and maintaining high standards of patient safety.
Responsibilities
- Perform nursing assessments on all patients within 8 hours of admission
- Develop individualized care plans based on patient assessment findings
- Collaborate with interdisciplinary teams to support patient care goals
- Maintain accurate and timely documentation in the medical record
- Deliver direct patient care using the nursing process (assessment, planning, implementation, and evaluation)
- Initiate discharge planning beginning on the day of admission
- Provide patient and family education related to care and recovery
- Communicate changes in patient condition to physicians and care teams
- Supervise and delegate tasks to ancillary nursing personnel when appropriate
- Maintain personal and environmental safety according to hospital policies
- Participate in team-building and professional development activities
- Assist with interdisciplinary conferences and patient rehabilitation planning
- Support continuity of care by communicating patient status updates to healthcare team members
