If you’re passionate about providing exceptional hospice care and want to work in a collaborative, supportive environment, this is the opportunity you've been waiting for!This position requires that you are available to work on Sundays 11am - 7pm with other possible per diem options throughout the week!Must be willing to cover all of Orange County and South Bay LA areas!Hospice Admissions RN Responsibilities:Participation in the information gathering, analysis, education and form completion aspects of the admissions processEvaluate patients for hospice appropriateness, e.g. medical records review as requested.Obtain agreement for admission of referred patients and families who meet admission criteria.Build effective referral source relationships to support the high quality and responsiveness expectations associated with Salus HospiceMaintain clinical and professional competence.Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice care.Hospice Admissions RN Qualifications:Registered Nurse with valid CA state licenseMinimum one year of hospice or home health experienceStrong background in medical/surgical, ICU/CCU, or ER is helpfulValid driver’s license, auto insurance, reliable transportation and willingness to drive to patient’s place of residenceMust have excellent interpersonal skills and a passion for hospiceUnderstanding of hospice philosophy and needs of the terminally illSelf-directed with ability to work with little supervisionCurrent Physical and TB test resultsHospice Admissions RN Benefits:Medical/Dental/Vision - FULL TIMEMileage Reimbursement401 K40 hours of Sick Leave per yearVacation Time - FULL TIMEHoliday Pay - FULL TIMEDiscounted Gym MembershipVerizon Wireless DiscountCorporate Discounts on Entertainment and Dining, etcPlease apply today!
