Saline Care Nursing & Rehabilitation is seeking a motivated and detail-oriented Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our leadership team as our MDS Coordinator / Nurse Manager.
This key leadership position is responsible for coordinating the MDS process, supporting nursing operations, improving resident outcomes, maintaining regulatory compliance, and promoting clinical excellence throughout the facility.

Compensation
Registered Nurse (RN): $33.00–$35.00 per hour (Based on Experience)
Licensed Practical Nurse (LPN): $29.00–$31.00 per hour (Based on Experience)
Sign-On Bonuses
Registered Nurses: $15,000
Licensed Practical Nurses: $10,000
Paid out over one year

Position Responsibilities
Coordinate and oversee the MDS assessment process.
Complete accurate and timely MDS assessments and submissions.
Ensure compliance with all federal and state regulations.
Collaborate with the interdisciplinary team to develop and maintain individualized resident care plans.
Monitor quality measures and support reimbursement optimization.
Assist with clinical documentation, survey readiness, and quality improvement initiatives.
Support nursing leadership while promoting exceptional resident care and regulatory compliance.
Qualifications
Current Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing.
Strong organizational and documentation skills.
Excellent communication and leadership abilities.
Ability to manage multiple priorities and meet deadlines.
Long-term care experience preferred.
Previous MDS experience is preferred but not required—we are willing to train the right candidate.
