POSITION SUMMARY
Provides skilled nursing care through assessment, care planning, delegation, and oversight of resident needs in accordance with regulatory and organizational standards. Coordinates care with residents, families, providers, and the care team to promote continuity, safety, and satisfaction while identifying and addressing changes in condition or concerns. Supports nursing staff through wound care education and coaching and maintains accurate documentation, confidentiality, and compliance with established policies and procedures.

ESSENTIAL JOB FUNCTIONS

QUALITY
Demonstrates competency in clinical skills and applies sound critical thinking in the delivery and delegation of resident care.
Conducts resident care assessments, including MDS assessments, in accordance with regulations, guidelines, and organizational standards.
Develops and supports consistent implementation of individualized resident written plans of care, including timely communication of changes to resident status.
Provides training and coaching to nursing staff related to wound care practices, interventions, and standard.

ACCESS
Coordinates care with residents, families, providers, and other staff members to promote effective communications, continuity of care, and patient and family satisfaction.
Identifies and communicates resident or family concerns, changes in condition, and unsafe conditions to appropriate members of the care team.
Attends and constructively contributes department meetings.

STEWARDSHIP
Completes and maintains required documentation and reports accurately and timely in accordance with organizational policies, guidelines, and protocols.
Uses appropriate professional judgement and follows established policies and procedures when providing care, delegates as appropriate.

POSITION QUALIFICATIONS

MINIMUM EDUCATION
Graduate of an accredited school of nursing

MINIMUM EXPERIENCE
None

REQUIRED REGISTRATION/LICENSE/CERTIFICATION
Current Kansas Registered Nurse license
Applicable Resuscitation and other position-related certifications must be obtained within the timeframe established by SRHC policy.
