Job Summary:

The Clinical Lead coordinates and oversees activities of the clinic and clinical team members to ensure compassionate, effective, and efficient delivery of high-quality clinical care for patients under the direction of the Director of Nursing.

Essential Job Functions:

  1. Manages and administers nursing services within households or the rehab unit according to Salemtowne procedures and policies.
  2. Assigns, delegates, supervises, and evaluates nursing professionals, as well as others involved in the household team.
  3. Assesses, plans, implements, and evaluates resident’s physical condition when necessary.
  4. Participates in documentation and completion of MDS Collaborating to provide best practices for the resident within the multidisciplinary team.
  5. Assists all members of the multidisciplinary team to achieve the resident’s optimum level of functioning.
  6. Develops the nursing component of the resident’s plan of care.
  7. Communicates with the residents and their representatives and develops positive relationships with them
  8. Communicates resident, family, team member concerns to the Director of Nursing in a timely manner.
  9. Models all aspects of employee behavior and appearance addressed in the Salemtowne Team Member Handbook
  10. Promotes safety awareness for both resident and employees.
  11. Attends and participates in Neighborhood Council and Clinical Lead meetings.
  12. Performs other duties as necessary and in compliance with the scope of practice.
  13. Contributes to the performance evaluation for licensed personnel.
  14. Interviews, orients, and selects licensed nurses for the household.
  15. Ensures timely call bell response.
  16. Participates in the nursing on call schedule.

Willing to do any tasks needed, including those that may seem menial or unpleasant. When communicating with physician or other medical personnel advocates for elder preferences. Refrains from pressuring or manipulating elders into accepting medication or treatment for the sake of convenience.
  • Critical Thinking: Recognizes early signs of changing conditions and uses all available and appropriate resources to resolve needs. Very strong observational and assessment skills with regards to physical/social/spiritual conditions, coupled with ability to discern effective response / treatment. Able to embrace uncommon methods if they will best suit elders’ needs.
  • Taking the Initiative: Sensitive to changing medical needs and the nature of staff responses and takes appropriate actions regardless of whether the needs have been reported by others.
  • Learning and Teaching Centered: Able to model person-centered practices and provide direction to other team members as needed.
  • Community Builder: Able to facilitate sense of team connection and participation. Able to nurture sense of accountability to elders and household among nursing staff. Able to advocate for needs of either household, elders, family, or staff as needed.
  • Conflict Facilitator: Able to mediate conflicts concerning team members as needed.
  • Interpersonal Sensitivity and Communication Skills: Able to provide clear directions, expectations, and feedback. Able to elicit trust and cooperation from team members. Able to communicate elders’ medical needs clearly with team members, resident representatives, and physicians.

Education and Experience:

  • Graduate of an accredited nursing school.
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing with NC Board of Nursing.
  • Current CPR certification or the ability to become CPR certified.

Physical Requirements:

  • Capable of standing and walking short distances for extended periods of time.
  • Ability to lift, carry, push and/or pull objects more than 50 pounds.
  • Stoop and bend at the knees, twisting from side to side at the waist.
  • Reach above, below, and at shoulder level.

Work Environment:

Works in a clean, well-lighted facility. Possible work hazards include bloodborne pathogens, moving parts, muscular pains, and strong offensive odors.
