Hours: Full time, part time, and PRN available. Day Shift: 6a-2p. Evening Shift: 2p-10p. Night Shift 10p-6a. 12-hour shift options available as well: 6a-6p, 10a-10p, 6p-6a
Responsibilities
- Assist patients with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support
- Maintain accurate medical documentation using Electronic Health Record (EHR) systems and ensure compliance with HIPAA regulations to protect patient confidentiality. This facility utilizes Point Click Care.
- Assist with patient transfer methods utilizing equipment such as Hoyer lifts or transfer belts to ensure safety during movement
- Provide companionship and emotional support to patients with dementia or experiencing end-of-life care needs in accordance with best practices in hospice and memory care.
Requirements
- Valid CNA certification from an accredited program; active license required
- Proven experience working with geriatric patients or individuals with developmental disabilities and dementia care experience preferred
- Familiarity with EMR/EHR systems for accurate medical record keeping
- Ability to assist with patient transfers safely using appropriate techniques and equipment such as Hoyer lifts
- Strong communication skills for effective client interaction and team collaboration
- Understanding of HIPAA guidelines to maintain patient privacy and confidentiality at all times
- Prior experience in long term care facilities is highly desirable
- Ability to work independently or as part of a team in fast-paced environments while maintaining compassionate patient care
