Your Role With Us:
Primary Care Resource Coordinator
Primary Functions:
1. Patient Resource Coordination (70%)
- Works with care teams and patients to address non-clinical barriers to care (e.g., transportation, food, housing, finances).
- Follows up with patients to ensure support resources are effective.
- Acts as an expert on community resources and keeps care teams informed.
- Attends interdisciplinary team meetings as needed.
- Documents coordination efforts in the electronic medical record.
2. Outreach Coordination (10%)
- Contacts patients due for preventive care based on guidelines and initiatives.
- Sends bulk communications and schedules appointments.
- Tracks and reports on outreach effectiveness.
3. Primary Care Clinic Support (20%)
- Provides backup for medical office tasks during busy periods (registration, calls, scheduling).
- Manages work queues and registration denials.
- Assists patients with MyChart and performs follow-up calls.
- Supports special projects and schedules preventive visits.

Background/Expertise:
Education:
- High School Diploma or Equivalent required. Advanced degree preferred.
Experience:
- Minimum of 1 year medical office experience in ambulatory clinic setting preferred.
- Traditional Health Worker, Community Health Worker, or Patient Navigator training preferred.
Special knowledge, skills, & abilities:
- Understands and can apply care management and coordination concepts, methods, and tasks.
- Ability to apply eligibility requirements, regulations, and policies and procedures to care coordination activities.
- Expert knowledge of community resources.
- Ability to connect with diverse populations to address unique needs.
- Strong interpersonal and communication skills, utilizing sensitivity, confidentiality and tact
- Excellent computer skills including strong expertise in Microsoft Office, PC based spreadsheets and electronic health records.
- Must be self-directed and able to exercise judgment in prioritization. Must work with high degree of accuracy.
