Salem is looking for an experienced professional RN with SNF or Home Health experience to provide healthcare services to our residents in our homes in residential neighborhoods in Chino and Ontario. Summary/Objective
Reporting to the Registered Nurse/Nursing Supervisor, the Registered Nurse is responsible for the health and healthcare services of residents in our Intermediate Care Facility programs for individuals who have intellectual disabilities. Perform quality assurance tasks; ensures that the program staff has the tools necessary to provide health and safety to residents.
Duties & Responsibilities
- Interact, work, and care for the residents using the Salem Direct Care Staff Standards of Care.
- Provide nursing services as required by regulations for assigned programs.
- Assess residents’ health needs and coordinate community medical resources to obtain services needed.
- Develop a health care plan, review semi-annually (or as needed) and write monthly and quarterly progress notes.
- Write monthly, semi-annual, and annual client health summaries.
- Review consultant assessments and reports.
- Assist in identifying and securing the necessary equipment to meet the nursing/health care needs of residents.
- Train staff in areas of medication administration, vital signs, infection control, seizure care, first aid, Restricted Health Care Plans, and signs/symptoms that warrant nursing or medical intervention.
- Maintain and review reports of residents’ medical histories and monitor changes in their conditions.
- Monitor medications: review documentation for residents, review drug regimen, and medication orders of each consumer monthly, and document in a file.
- Review files for follow through on physician orders and review of lab work.
- Notify the attending physician of any sudden and/or adverse change in symptoms or behavior exhibited by a resident.
- Participate in interdisciplinary team meetings, Human Rights Committee, Pre-admission screening, New Admissions, Department of Public Health Surveys, and discharge planning.
- Ensure all health care is provided per Title 17 and Title 22 requirements, established policies and procedures.
- Provide medication training for all staff within 30 days of hire, retraining for any assigned staff, and in-service training to Salem staff as needed.
- Visit residents who are hospitalized in all programs.
- Attend Program meetings, as well as attend all required training, staff meetings and regional Center meetings. Complete required E-learning coursework by due dates.
- Maintains communication with appropriate agencies and participates in community organizations to advocate for the organization and Salem residents.
- Perform all other duties and responsibilities as assigned by supervisors.
- May be required to work overtime.
Qualifications
Travel Required: 100 % up to 30 miles to various facilities, meetings, and hospital visits. Willingness to drive personal and/or rental vehicle, when appropriate for business purposes.
Hours Required: This is a full-time 30-hour position with various shifts.
Education Required: Bachelors degree in Nursing or Health Care Management. An active and maintenance of California Registered Nurse License.
Experience required: A minimum of 3 years’ experience providing clinical support in a developmentally disabled youth or adult setting. Familiarity with developmentally disabled healthcare laws, regulations, and standards. Must meet Title 17 and/or Title 22 requirements including, but not limited to: Minimum age 21. Bi-cultural/bi-lingual experience is an asset. Strong computer skills for using Microsoft Office Suite, Office 365, Electronic Health Record System, Internet, and related business applications.
Skills and Abilities: Ability to plan, organize, and work in a specialized residential services program. Provide professional consultation and leadership. Be able to recognize and deal effectively with a wide variety of personalities and situations requiring tact, poise, friendliness and sensitivity. Ability to think clearly and manage multiple changing priorities while using good judgment. Be able to maintain integrity and confidentiality regulations in regard to client records. Must be able to communicate well in English, both verbally and in writing. Ability to communicate verbally by phone in a concise and clear manner.
Physical Abilities: Ability to lift up to 50 lbs. (to transfer clients). Must be able to navigate several flights of stairs during each work shift. Ability to sit at a desk and work with a computer approx. 8 hours a day. Ability to drive in heavy traffic and inclement weather.
License Required: A valid California driver's License - Class II/C. Proof of current auto insurance. Current RN License.
