SAJE Strategic Solutions is seeking a dynamic, highly organized, and results-driven Director of Rehabilitation (DOR) to oversee therapy operations across two premier post-acute/skilled nursing facilities (SNF) in the Greater Los Angeles area. Operating under a single owner/operator, this unique leadership role splits time equitably between both buildings to ensure clinical excellence, regulatory compliance, and optimal caseload management across both locations. This full-time role offers a base salary of $135,000 – $148,000.
The ideal candidate is a licensed clinician (PT, OT, or SLP) with strong leadership acumen who excels at multi-site logistics, building collaborative relationships with facility administrators, and mentoring interdisciplinary therapy teams.
Key Responsibilities- Multi-Site Operations: Direct and manage the daily operations of the rehabilitation departments across two designated LA-area facilities, splitting time effectively based on building census, clinical needs, and operational priorities.
- Team Leadership & Staffing: Recruit, mentor, and schedule physical, occupational, and speech therapists (including assistants and aides) to ensure appropriate staffing levels and peak team performance at both sites.
- Clinical Oversight: Guarantee the delivery of high-quality, patient-centered care. Review documentation, conduct audits, and ensure strict adherence to state and federal guidelines, including Medicare parts A and B.
- Utilization Management: Oversee case management, tracking therapy minutes and utilization to balance clinical efficacy with operational efficiency.
- Interdisciplinary Collaboration: Serve as the primary liaison between the therapy departments, facility administrators, Directors of Nursing (DONs), and physicians to ensure seamless, integrated patient care.
- Licensure: Active, unrestricted California license as a Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP) is required.
- Experience: Minimum of 2–3 years of experience as a Director of Rehab (DOR) or Assistant DOR, preferably within a skilled nursing (SNF) or long-term care setting. Experience managing multi-site operations or split-campus environments is highly valued.
- Industry Skills: Comprehensive knowledge of PDPM, Medicare regulations, and California state healthcare compliance. Strong organizational, travel-management, and communication skills are required.
- Salary Range: $135,000 – $148,000 base salary.
