Direct Support Staff/Direct Support Professionals SUPPORT STAFF DUTIES Support staff are the most important asset of the Ste. Genevieve Group Home, and of the people that we serve. The most important aspects of the position are supporting residents in living as independently as they can, and in becoming an integral part of their community. The various components of the job are as follows: 1) Medical/Personal Care A. Administer medications as ordered. 1. Follow procedures as specified in the medication administration course 2. Is trained in and prepares food according to special diets ordered by clients pcp. B. Transport residents served medical appointments. 1. Take a physician’s orders form to every appointment. Assure that the top section is completed before the appointment with the resident’s name, date of birth, Medicare number, Medicaid number, allergies, date of the visit, reason for the visit, and signature of the support staff. Make sure that the physician or clinician signs and dates the form. 2. Give the completed form to the RMA immediately. 3. Order and pick up any meds as needed. 4. Inform the RMA of all appointments made for the resident. C. Monitor the health status of the resident and inform the supervisor or the physician of any negative changes in health status. D. Provide any personal care as necessary, based on the needs of the resident. This may include bathing, dressing, toileting, hair care, oral hygiene, etc. E. Assure that the resident’s appearance is as neat as possible, that clothing is nice and clean, and in good condition. Assist the resident in choosing appropriate clothing and assure that the clothing is on correctly. 2) Housekeeping A. The resident’s home, specifically their room will be kept clean and neat. Support staff will encourage the resident to keep their room neat and clean. Support staff will assist the resident in completing cleaning tasks and may need to actually do the cleaning if necessary. Support staff will make sure that the home is neat and clean at the end of their shift and will not leave cleaning unaccomplished for the next support staff to complete. B. Support staff will assure that the resident has quality meals that comply with any diet that they may follow. Support staff will monitor the person during meal preparation, assisting as necessary. Staff may have to complete all meal preparation, depending on the needs of the resident. C. Support staff will assist the resident in completing laundry tasks, assuring that the resident follows proper procedures. Support staff may need to complete all laundry tasks, depending on the needs of the resident. D. Support staff will assure that the resident’s home is in a safe condition. Look at the home when arriving for the shift and look for any unsafe conditions. Correct any situations that can be corrected in a safe and easy manner. If conditions exist that are dangerous and not easily and safely corrected, remove the resident from the situation. Contact the Administrator or the PM and follow their instructions. Do not leave the resident in a dangerous situation. 3) Personal Plan A. Each resident has a Personal Plan. It is the responsibility of each support staff to read the Personal Plan and be familiar with all aspects of the plan. This plan contains much useful information about the resident and techniques for working with him/her. The Personal Plan will also contain information about diets and about any medical concerns and procedures. B. Support staff is responsible for addressing objects of the plan and for documenting the resident’s progress on plan objectives. C. Support staff assists in development of the Personal Plan by providing input and providing information for the plan. D. Support staff shall be present during visits by Department of Mental Health or Sikeston Regional Center staff should be prepared to give information about the resident’s progress on the Personal Plan. 4) Recordkeeping A) Support staff will make a case note entry at the end of every day. The case note will show the date, the shift time (2-10 for example), what transpired during the shift, and the signature of the support staff. All case notes will be completed with black ink. More frequent case notes may be necessary if unusual events occurred during the shift. B) Support staff will safeguard the resident’s records during their shift, assuring that no person accesses the records without being authorized. No information will be released to anyone outside of Ste. Genevieve Group Home without utilizing proper confidentiality procedures. 5) Community Integration A) Support staff will assist the resident as needed to be a part of the community by involving him/her in community events, by going out for shopping or recreational activities, transporting to church, transporting to social events, or any other activities that may be of interest to the resident. B) Support Staff are required to offer activities approved by the administrator. 6) Support staff will follow any other directions and instructions as provided by supervisory staff for activities that may not be listed above. 7) Support staff on shift are not to leave unless approved by administration. Minimum age and educational requirements: Eighteen years of age and high school diploma or equivalent. All staff agree to complete the following mandated training annually: First Aid, CPR, Positive Behavioral Support, Abuse and Neglect, Confidentiality, Person Centered Planning, and semi-annually: Medication Administration . These trainings are provided by the agency. A Class E Driver’s License is also required. Additional program training may be necessary and will be scheduled. Additional duties of Direct Support Professionals not listed above may include, but are not limited to, the items as follows: Observes any changes in each resident’s physical condition, behavioral responses, and emotional status, and records such changes for each shift on a daily basis. Any significant changes shall be reported directly to the Administrator and or PM. Provides basic health care for residents, as directed. This includes, but may not be limited to, administering medication, taking and recording of body weight, body temperature, pulse, respiration, blood pressure, and the application of first aid procedures, when appropriate. Assists in the implementation of individual resident and/or group program plans as approved and directed by the PM. Implements an individual resident program that may consist of activities which promote community integration goals such as shopping, volunteer work, and recreation. Direct Support Professionals, therefore, may be required to oversee resident spending and the removal of resident funds from each respective resident’s “money box”. Maintains an inventory of supplies and equipment, and orders replacements when needed, as directed by the PM. Assists in daily living tasks that may include, but are not necessarily limited to, housekeeping, dressing, bathing, and toileting (when appropriate). Takes appropriate action, as determined by standard policies and adopted procedures, in the event of a fire/natural disaster/emergency situation. Performs other related job tasks, as assigned. Follows proper chain of command.
