Saint John 23rd Home is seeking a knowledgeable and detail-oriented RNAC to join our team on a part-time basis. The ideal candidate is an experienced RN with strong MDS (Minimum Data Set) assessment skills, excellent communication abilities, and a commitment to resident-centered care.
As the RNAC, you will oversee and coordinate the Resident Assessment Instrument (RAI) process to ensure timely, accurate, and compliant MDS submissions. You will collaborate closely with nursing, therapy, finance, and interdisciplinary team members to support high-quality resident care and optimal reimbursement.
Key Responsibilities
- Complete and manage MDS assessments in accordance with federal/state regulations and facility policy
- Ensure accuracy of all assessments, coding, and documentation
- Coordinate the RAI process and serve as the MDS resource for the care team
- Work with the interdisciplinary team to develop individualized care plans
- Track and manage assessment schedules to meet all deadlines
- Participate in Quality Assurance and Performance Improvement (QAPI) activities
- Maintain compliance with PDPM and all regulatory requirements
Qualifications
- Active, unencumbered RN license
- Minimum 1 year MDS/RNAC experience preferred (PDPM experience strongly preferred)
- Skilled Nursing Facility experience required
- Strong understanding of Medicare/Medicaid guidelines
- Excellent attention to detail, organizational skills, and communication
- Ability to work independently and manage deadlines
