We at Saint John 23rd Home are seeking a dependable, Resident-focused Licensed Practical Nurse (LPN) to support our Personal Care Home Residents with dignity, compassion, and professionalism. The Full-Time LPN will be responsible for delivering high-quality care to residents, administering medications, coordinating with physicians, and ensuring that residents receive safe and effective clinical support in accordance with all regulations and facility policies.
- FULL-TIME LPN HYBRID (Day / Afternoon Shifts)
- 5 Days per Week

Responsibilities:
- Provide direct nursing care and supervision to residents in a personal care setting
- Administer medications and treatments in accordance with physician orders
- Monitor residents for changes in condition and report promptly
- Maintain detailed, accurate documentation
- Assist with resident assessments and individualized service plans
- Communicate effectively with residents, families, and interdisciplinary team
- Ensure compliance with state regulations and facility standards
- Support caregivers and promote a positive teamwork environment
Qualifications:
- Current and valid LPN license in the State of Pennsylvania
- Prior experience in personal care, assisted living, long-term care, or similar setting preferred
- Strong communication, assessment, and organizational skills
- Compassionate, patient, and resident-centered mindset
- Ability to work independently and collaboratively
