ESSENTIAL DUTIES:
Clinical Operations & Oversight:
- Assist the DON in daily clinical operations across SNF and AL
- Provide supervision and support to nurses and nursing assistants
- Conduct routine rounds (clinical, environmental, and quality of care)
- Monitor changes in resident conditions and ensure timely interventions
- Support admissions, transfers, and discharges
Regulatory Compliance & Risk Management
- Maintain compliance with CMS, WI DHS regulations, and organizational policies.
- Participate in survey readiness activities and accompany surveyors as assigned.
- Perform investigations, ensure timely incident/accident reporting, and participate in root‑cause analysis.
- Monitor nursing documentation for accuracy and compliance.
Quality Assurance & Performance Improvement (QAPI):
- Assist in planning, implementing, and monitoring QAPI activities.
- Track and analyze clinical indicators (falls, infections, wounds, hospitalizations).
- Conduct audits and follow through on corrective action plans.
- Participate in Performance Improvement Projects (PIPs) as assigned.
Staff Leadership & Development:
- Provide leadership, coaching, and mentoring to staff across shifts.
- Participate in interviewing, hiring, onboarding, and retention efforts.
- Support progressive discipline and performance evaluations as delegated.
- Assist with developing and teaching in-service education and clinical competencies.
Communication & Coordination:
- Serve as a liaison between staff, residents, families, providers, and leadership.
- Support service recovery and family concerns.
- Promote teamwork, communication, and conflict resolution practices.
Staffing, Scheduling & Resource Management:
- Assist with developing staffing patterns and ensuring PPD compliance.
- Monitor attendance, overtime, and staffing concerns.
- Support the staffing coordinator in daily scheduling needs and emergency coverage
Infection Prevention Support:
- Support infection control protocols and surveillance practices.
- Assist the Infection Preventionist during outbreaks and monitoring activities.
- Ensure staff compliance with PPE, hand hygiene, and standard precautions.
Supervisory Responsibilities:
- Supervises nursing staff as delegated by the DON, including RNs, LPNs, and RAs.
- Provides leadership support to Nurse Managers, Supervisors, and other clinical leaders.
- Acts as Director of Nursing in the DON’s absence.
- Participation in the rotating on‑call schedule is required.
- Other duties as assigned.
