Job Description:
We are seeking a passionate and motivated individual to join our team as a Social Worker in the inpatient hospital setting. As a member of our healthcare team, you will provide advocacy, education, assessments, and interventions to patients and families to achieve an optimal level of care.
Key Responsibilities:
- Develop and implement comprehensive treatment plans in collaboration with medical staff and outside organizations.
- Cultivate strong relationships with patients, families, and caregivers to ensure seamless transition planning and quality clinical outcomes.
- Provide brief counseling, supportive therapy, crisis intervention, grief counseling, and referral to alternative community resources as needed.
Requirements:
- Master's Degree in Social Work required.
- Two years of experience in hospital setting or medical social work setting preferred.
- Licensed Clinical Social Worker (LCSW) preferred, but Master Social Worker (LMSW) is required.
