Position Summary:

This position is responsible for providing case management services to victims of abuse and/or assault. This includes, but is not limited to, linking and coordinating of services, crisis intervention, and advocacy. The Case Manager also provides outreach services throughout the community by participating in committees and providing education and outreach trainings, presentations, and awareness activities. This is a grant funded position.

Essential Duties and Responsibilities:

  • Provide crisis intervention and support to clients.
  • Complete screening and intake process.
  • Coordinate, monitor, and advocate for appropriate services.
  • Provide and/or arrange transportation services for clients.
  • Provide education and information to clients as directed by needs.
  • Participate as a member of NMN clinical team that meets weekly to present cases for clinical feedback and direction.
  • Serve on the Sexual Assault Response Team (SART); participate in after-hours on call rotation.
  • Develop, arrange for, monitor and evaluate client progress according to measurable and date-specific goals in the clients individualized case management plan.
  • Conduct home visits when appropriate.
  • Coordinate with Tribal Police, Victims of Crime Advocate, and other law enforcement agencies as requested.
  • Participate in community care coordination teams as appropriate.
  • Serve as facilitator or co-facilitator for therapeutic or support groups.
  • Collaborate with community sexual assault and domestic violence service providers.
  • Capable of public speaking, conducting formal and informal meetings and conferences.
  • Complete documentation of client interactions as required and per policy.
  • Maintain statistical records and proper documentation on clients served.
  • Track necessary data for grant and complete grant reporting as required.
  • Participate in outreach and education services within the community.
  • Coordinate community activities to enhance community awareness.
  • Maintain professional and technical knowledge by attending educational workshops and establishing networks with other service providers that focus on providing services to trauma survivors.
  • Adhere to client confidentiality and comply with department policies and federal regulations under the Privacy rules of the Health Insurance Portability and Accountability Act (HIPAA) and Violence Against Women Act (VAWA) Confidentiality Provision.
  • Provide excellent customer service for all internal and external customers of the operations at all times. Provide solutions for customer concerns and continually focus on customer service as our top priority.
  • Must maintain strict confidentiality and present a positive, professional demeanor and image at all times. Learn Emergency Policies and Procedures and participate in required drills.
  • Perform all work duties and activities in accordance with Tribal Operations & Behavioral Health policies and procedures.
  • Demonstrate a commitment to cultural programming by learning of cultural teachings, participating in and coordinating cultural activities for clients, and by providing continuous support for the enhancement of the culture, language, and traditions.
  • Demonstrate behaviors gifted and described by the 7 Grandfather Teachings.
  • Demonstrate knowledge of the Recipient Rights Advisor processes and refer clients as applicable.

Contacts/Purpose of Contacts:

  • Contact with tribal youth, adults, families, general public, social services, other agencies and providers deemed appropriate to accomplish effective services and coordination of care.

Minimum Qualifications:

  • Bachelors Degree in Social Work, Psychology, or Human Services related field.
  • Must be able to pass background check to meet the employment eligibility requirements as they pertain to the position.

Desired Qualifications:

  • One-year experience working with victims of domestic violence, sexual assault or stalking.
  • Experience with federal grant reporting, tracking, and evaluation.

License, Certification, or Special Requirements:

  • CPR/First Aid Certification upon 6 months of hire.
  • Obtain HIPAA certification within five business days of hire.
  • Must have a valid Michigan Drivers License and must be able to obtain and maintain a Tribal Drivers License and qualify for coverage by SCIT auto liability insurance carrier.
  • Native American preference shall apply to all positions.

Knowledge, Skills, and Abilities:

  • Knowledge of domestic violence, sexual assault, and/or stalking.
  • Knowledge of community outreach services and activities.
  • Skill in preparing and maintaining patient records.
  • Skill in tracking, reporting, and maintaining reports.
  • Ability to develop and present educational programs and workshops.
  • Ability to build rapport quickly and effectively with clients and their families.
  • Ability to integrate training, experience, and common sense sufficient to identify potentially dangerous situations to resolve the situation with minimum injury to self or other persons.
  • Ability to establish and maintain professional relationships with individuals of varying social and cultural backgrounds and with co-workers at all levels.
  • Ability to handle multiple tasks and meet deadlines.

Physical Demands:

  • Frequent use of hands, wrists, fingers associated with computer equipment.
  • Required to sit for extended periods of time.
  • Occasionally walk and stand.
  • Normal visual acuity, ability to talk and hear.
  • Occasionally lift and/or move up to 20 pounds.

Work Environment:

  • Normal office environment.
  • High stress level as associated with the demands of crisis interventions and emergencies is encountered as a result of challenges in the behavioral health field.
  • Mandated reporting of abuse and neglect can lead to personal challenges with exposure to victims of abuse.
  • High incidence of exposure to communicable diseases and blood borne pathogens.
  • Extended hours and irregular shifts may be required.
  • Travel may be required.
