Position Summary:
Under the supervision of Program Manager, serve as a community liaison for tribal members assisting with outreach, coordination, follow-up, referrals and direction to all available tribal member programs and services. Adhere to patient confidentiality and complies with clinic policies and federal regulations under the Privacy rules of the Health Insurance Portability and Accountability Act (HIPAA). All Saganing Outreach staff must adhere to mandated reporting laws.
Essential Duties and Responsibilities:
- Assist with the planning and coordination of on-site activities in special program area including fitness and guest speakers from service agencies.
- Assist with providing information and distributing correspondence relating to a variety of services; assist with distributing applications for assistance.
- Work with local service providers outside the Saginaw Chippewa Tribal community to advocate for the special needs and to promote culturally within the Program and through the Saginaw Chippewa Indian Tribal service system.
- Educate Tribal members of the responsible use of program funds and resources, both within the program and through the Saginaw Chippewa Indian Tribal service program.
- Provide transportation to members for medical appointments as needed.
- Maintain kitchen and inventory of supplies and food prep for all events hosted by the Program.
- Assist in janitorial duties to maintain the cleanliness of the building.
- Keep current on all elder issues and information including Medicare, dementia and elder abuse.
- Adhere to patient confidentiality and complies with clinic policies and federal regulations under the Privacy rules of the Health Insurance Portability and Accountability Act (HIPAA).
- Provide excellent customer service for all internal and external customers of the operations at all times. Provide solutions for customer concerns and continually focus on customer service as our top priority.
- Must maintain strict confidentiality and present a positive, professional demeanor and image at all times.

Contacts/Purpose of Contacts:
- Contact with Saganing Outreach Program staff, members of the organization, tribal members and members outside of the community, including tribal youth.
- Contact Social Service Agencies for care, information and referrals.
- Contact with other Departments within the organization to provide all available services.
Minimum Qualifications:
- Must be at least 18 years of age.
- High School Diploma or GED.
- Experience with Microsoft Office programs.
- Must be able to pass background check to meet the employment eligibility requirements as they pertain to the position.
Desired Qualifications:
- Associate’s degree or two years college education in a human service area.
- Experience with Saganing tribal service programs as well as outside service organizations.
- Experience with Native American communities and elder populations.
License, Certification, or Special Requirements:
- Ability to obtain HIPAA Certification, Blood Borne Pathogens Certification, and Safety Training within probationary period.
- Must have a valid State of Michigan Driver’s License with the proper designation required for the vehicle utilized to perform the job duties; must be able to obtain and maintain a Tribal Driver’s License and qualify for coverage by SCIT auto liability insurance carrier.
- Native American preference shall apply to all positions.

Physical Demands:

Work Environment:
- Normal office environment, outdoors and home setting.
- Must possess sufficient stamina to work under all types of weather conditions including extreme hot/cold and to lift, carry, push, or pull a variety of tools, equipment and materials.
- May be required to work other shifts, nights, weekends, holidays, and during emergency situations.
- Extended hours and irregular shifts may be required.
- Travel may be required.
