What You’ll Do:
As a Health Services Coordinator (LPN), you’ll play a hands-on role in improving the lives of our residents by:
- Conducting comprehensive resident assessments prior to move-in, re-entry, and on an ongoing basis to ensure all physical, mental, and social needs are met.
- Assisting with community readiness for regulatory inspections, including maintaining accurate service plans, medication records, and emergency preparedness.
- Establishing and fostering positive relationships with residents, families, associates, home health, hospice, and other external partners.
- Coordinating and ensuring proper health-related training and education for the caregiver team, conducting skill observations to optimize resident care delivery.
- Providing direct resident assistance as needed, including personal care activities and administering medication in accordance with physician orders.
What We’re Looking For:
- An active Nursing license with the state.
- A current CPR/BLS certification.
- 1 years of experience in a senior living or related industry supervisory or leadership role.
- 1 years of experience providing care to cognitive and memory-care residents or patients (preferred).
- Valid driver’s license and acceptable driving record; ability to provide resident transportation and/or serve alcoholic beverages at community events.
- The ability to assist residents with limited mobility, lift/push/pull up to 50lbs.
- Excellent verbal and written communication skills, with the ability to effectively collaborate with diverse groups, navigate challenging situations, and make sound decisions.
- Strong technical skills (e.g., Word, Excel, PowerPoint) and the ability to maintain confidential and privileged information (including HIPAA, residents, and associates).
