Case Manager (RN)

Essential duties include:

• Reviews insurance, governmental, accrediting agency and approved hospital healthcare management guideline standards to determine criteria concerning admission, treatment, appropriate level of care, and length of stay for patients.
• Performs utilization review functions of assigned cases.
• Compare inpatient medical records to established criteria and requests interdisciplinary feedback to determine appropriateness of treatment and level of care using pre-established criteria for intensity of service.
• Performs initial assessment for inpatient including: patient/family interviews to obtain full clinical/social knowledge of patient and identifies discharge planning needs.
• Participate in discharge planning and in-service programs related to the needs of the patient
• Great time management skills

Qualifications:

• Current Texas Registered Nurse license
• Bachelor's degree in nursing or healthcare administration preferred
• 3 years of acute care clinical nursing experience preferred
• Minimum of 1 year Case Management experience
• Previous Inpatient Rehabilitation or Skilled Nursing Facility experience preferred
• Good oral and written communication skills
