travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage
Reimbursement: $0.50/mile for distance
covered between patient home appointments

Shift: 8 Hours; Days, 8.00a - 5.00p
Position Requirements:
- Registered Nurse: 2
years - Required
- Recent/Current
Hospice Nursing Experience in a Home Setting: 1 year within the past 3 years – Required
- Home Care Home Base
(HCHB) charting experience: Recent/current - Required
- Must be Covid
Vaccinated - Declination not accepted for this position, may accept medical/religious exemptions on case by case basis
Education, License
and Certification Requirements:
- ASN or an equivalent
– Required
- BSN or an equivalent
– Highly preferred
- RN License for State applying or compact if accepted in the state - Required
- BLS Certification

Job Description:
- Provide and
coordinate care for assigned Hospice patients in a home setting, facilitate case management, perform admission and revisits for the assigned case load.
- Develop and implement
nursing care plans for patients as assigned.
- Empower patients and
families to reach healthcare goals by educating them on disease management.
- Provide Home Health nursing
care and services within the standards and scope of nursing practice as
defined within the state Nurse Practice Act regulations, policies, procedures,
and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’
orders are legible if taken verbally and transcribed appropriately per
policy and procedure.
- Ensure patients’
status is assessed on an ongoing basis and pertinent information is
gathered and documented.
- Notify physicians of
significant changes in status.
- Document and complete
charting
- Territories: Lafayette, IN and surrounding areas
