Sage Group HealthCare Staffing is seeking a Home Health RN for a travel/contract position in New Lenox, IL. We are accepting both local and traveler candidates.

Pay Rate: $2200 Weekly Gross Pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Unit: Home Health 

Shifts available: Days

Schedule: 40 hours/week guaranteed, 5 shifts per week (M-F), Weekend, Call TBD

• Registered Nurse experience: 2 years - Required
  
• Home health nursing experience: 1 year (recent/current) - Required 
  
• Will accept candidates with strong Med-Surg experience if Home health experience is limited or not recent
  
• Matrix EMR charting experience - Highly preferred
  
• OASIS experience - Highly preferred
  
• Start of care experience - Highly preferred 
  
• Prior Travel Nursing experience – Preferred
  

Education, License, Certification/s and Other Requirements:

• ASN Degree or Diploma in Nursing - Required
  
• BSN Degree in Nursing - Preferred
  
• RN License for the State of Illinois – Required
  
• AHA BLS Certification  - Required
  
• Valid Driver License - Required
  
• Proof of Auto Insurance - Required
  
• Covid Vaccination Proof - Required 
  

Job Description:

• Performs all aspects of home health nursing for assigned patients in the comfort of their homes.
  
• Performs some virtual/tele
  
• Prescribes, coordinates and delivers care through the nursing process. Maintains the standards of professional nursing.
  
• Supervises CNA and coordinates care with other healthcare workers as assigned.
  
• Provides comprehensive initial assessments to all clients and on subsequent visits, continually assesses Physical, Psychosocial, Environmental, and community resources available utilizing the principles of growth and development over the life span
  
• Initiates the Plan of Care, re-evaluates the need for care, and modifies it as ordered/needed. 
  
• Establishes discharge plans with patients, physicians, other disciplines, and caregivers.
  
• Completes OASIS data
  
• Provides patient care using current Federal, State and Agency regulations
  
• Assesses the educational needs of patients, families, caregivers and implements appropriate teaching strategies
  
• Completes documentation according to regulatory guidelines.
  
• Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits
  
• Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct
  
• Perform some Telehealth/Virtual visits in addition to home visits
  
• EMR: Matrix - clinician will be provided with an iPad.
  
• Interview Process: Phone screen and in person or virtual interview with the clinical manager.
  
• Territory: Within 15 miles of New Lenox (Zip code 60451), may increase if census is low to meet guaranteed hours
  
• Weekly Case Load: 24-26 (Flexible depending on territory radius assigned)
  
• Orientation: 2 days. First day will be in the office. Second day out in the field
  
• Services offered by the facility: Catheter Care, Chronic Disease Management & Telemonitoring, Diabetes Management, Home Health Aide Services, Infusion Management (including antibiotic therapy), Laboratory testing, Medical Social Service, Nutrition Instruction, Physical, Occupational, and Speech Therapy, Skilled Nursing, Telemonitoring and Virtual visits, Tube Feeding, Wound/Ostomy Care
