Range $2560 to $2600 Weekly Gross pay based on a 40-hour work week (Includes Non-taxable
travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage
Reimbursement: $0.50/mile for the distance
covered between patient home appointments

Shift: 8 Hours; Days, 8.00a - 5.00p
Position Requirements:
- Current Physical Therapist experience: 1- 2
years - Required
- PT experience in a home health setting - 12 months within the last 3 years - Required
- Homecare Homebase (HCHB) charting - Required
- Must be comfortable to provide PT services for patients in a home setting - Required
Education, License
and Certification Requirements:
- Bachelor of Science in Physical Therapy or Similar - Required
- Current State License as Physical Therapist or Compact if applicable - Required
- BLS Certification
from AHA or American Red Cross - Required
Job Description:
- Provide skilled physical therapy
services to patients in their homes as ordered by the attending
physician.
- Reviews diagnostic data and its clinical relevance when treating
patients and considers current presentation of past medical/clinical history.
- Provides treatment interventions based on a treatment plan and goals
established for the patient.
- Observe, record, and report patient
reactions, changes, and progress to IDT team
- Monitors response/progress of assigned customers to treatment plan and
discussed with PT plans needing adjustments as necessary to meet changing needs
of the patient.
- Initiates patient care activities in reasonable/timely manner and
strictly adheres to department guidelines regarding a patient’s treatment
schedule.
- Treats patient to relieve pain,
develop or restore function, and maintain maximum performance using physical
means, such as, exercise, massage, heat, water, etc.
- Counsel patient and family on the
disease/injury and how to manage in all phases of physical therapy.
- Instruct patient and family members on
proper use of equipment such as wheelchairs, braces, walkers, crutches, canes,
and other prosthetic/orthotic devices.
- Oversee and supervise therapy services
of patient provided by PTAs and when appropriate, nurse assistants.
- Communicate with the PTAs and nurse
aides on supervisory visit results.
- Ensures patient charges are accurately captured and submitted within 24
hours of treatment
- Completes documentation that is accurate, complete, timely, legible,
and consistent with department documentation policies and regulatory agencies.
Performs other functions as assigned - Territories to cover: Perry and Morgan Counties
