Pay Rate (Days):
Range $1416 to $1440 Weekly Gross pay based on a 24-hour work week (Includes non-taxable
travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage
Reimbursement: $0.50/mile for distance
covered between patient home appointments
Unit: Home Health, Hospice
Shifts available: Days
Schedule: 36 hours/week guaranteed, 3 X 12-hr shifts, Friday, Saturday and Sunday of each week
Position Requirements:
- Registered Nurse: 1-2 years - Required
- Recent/Current
Hospice Nursing experience in a home health setting: 1 year within the last 3 years – Required
- Home Care Home Base
(HCHB) charting experience – Required
- Must have a valid
Driver’s License and Personal Auto Insurance
- Covid Vaccination (2 doses) Proof - Required, Declinations not accepted, Exemptions may be considered on a case to case basis
- Must be able to provide Immunization records or titers for MMRV and HepB vaccination upon offer prior to start
Education, License
and Certification Requirements:
- ASN or an equivalent
– Required
- BSN or an equivalent

- RN License for State
of Ohio or Compact - Required
- BLS Certification
from AHA or American Red Cross - Required
Job Description:
- Provide and
coordinate care for assigned hospice patients in comfort of their homes.
- Develop and implement
nursing care plans for assigned patient case load
- Support patient comfort and ability to stay at home - enabling them to spend quality time
with their loved ones and doing the activities they enjoy.
- Empower patients and
families to reach healthcare goals by educating them on disease management.
- Provide Hospice nursing
care and services within the standards and scope of nursing practice as
defined within the state Nurse Practice Act regulations, policies, procedures,
and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’
orders are legible if taken verbally and transcribed appropriately per
policy and procedure.
- Ensure patients’
status is assessed on an ongoing basis and pertinent information is
gathered and documented.
- Notify physicians of
significant changes in status.
- Document and complete
charting for all patient visits within 24 hours.
- Provide bereavement
support to patient’s families as needed.
