We are seeking to hire a LPN Home Health for in Fairfax, VA. We are accepting both local and traveler candidates.Position Details:Position Name: LPN Home HealthLocation: Fairfax, VirginiaSpecialty: Home HealthStart Date: ASAPEmployment Type: Full-time, Travel, Contract, TemporaryContract Length: 13 weeks contract Pay Rate: Range $1680 to $1720 Weekly Gross pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)Mileage Reimbursement: $0.50/mile for distance covered between patient home appointmentsUnit: Home HealthShift: 8 Hours; Days, 8.00a - 5.00pSchedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, Must meet assigned productivity points per weekPosition Requirements:LPN experience: 2 years - RequiredRecent/Current Home Health LPN Experience: 1 year within the past 3 years – RequiredHome Care Home Base (HCHB) charting experience: Recent/current - RequiredFamiliarity with OASIS in assisting RN's to complete the OASIS assessment - Required Must be Covid Vaccination with booster - No exemptions, booster can be obtained upon offer Education, License and Certification Requirements:LPN License for State of VA  - RequiredBLS Certification from AHA or American Red Cross - RequiredJob Description:Under general supervision, provide and coordinate care for assigned Home Health patients Empower patients and families to reach healthcare goals by educating them on disease management.Under general supervision, provide Home Health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.Under general supervision, ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.Under general supervision, ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.Notify physicians and/or supervising clinician of significant changes in status.Document and complete charting Territories: Fairfax & Prince William County area and 50 miles radius
