We are seeking Hospice Registered Nurse Case Manager for a travel / contract position in Columbus, Ohio. Accepting both local and traveler candidates.Position Details:Position Name: Hospice Registered Nurse Case ManagerLocation: Columbus, Ohio Specialty: HospiceStart Date: ASAPEmployment Type: Full-time, Travel, Contract, TemporaryContract Length: 13 weeks contract -Accepting Local and Travel CandidatesPay Rate: Range $2080 to $2120 Weekly Gross pay based on a 40 hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)Mileage Reimbursement: $0.49/mile for distance covered between patient home appointmentsUnit: Home settingShift available: DaysShift timings: 8-hour Days, 8.00 am to 5.00 pm Schedule: 40 hours/week guaranteed. 5 X 8-hr shifts, Mon-Fri Position Requirements:Registered Nurse: 2 years - RequiredRecent/Current Hospice nursing experience in a home health setting: 1 year within the last 3 years – RequiredHands on Home Care Home Base (HCHB) charting experience – RequiredOASIS experience - PreferredMust have valid Driver’s License and Personal Auto InsuranceMust be Covid Vaccinated or provide medical or religious exemptionEducation, License and Certification Requirements:ASN or an equivalent – RequiredBSN or an equivalent – Highly preferredRN License for State of Ohio or Compact - RequiredBLS Certification from AHA or American Red Cross - RequiredJob Description:Provide and coordinate care for assigned Hospice patientsComplete admissions, visits and manage assigned case loadDevelop and implement nursing care plans for patients as assigned.Empower patients and families to reach healthcare goals by educating them on disease management.Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.Notify physicians of significant changes in status.Document and complete chartingTerritories: Columbus, OH and surrounding areas
