About the Role:

The Registered Nurse-Home Health plays a critical role in delivering high-quality nursing care to patients within their homes, ensuring their health and well-being outside of traditional clinical settings. This position involves comprehensive assessment, planning, implementation, and evaluation of individualized care plans tailored to each patient's unique medical needs. The nurse will collaborate closely with interdisciplinary teams, patients, and their families to promote recovery, manage chronic conditions, and prevent hospital readmissions. By providing education and support, the nurse empowers patients to maintain optimal health and independence in their home environment. Ultimately, this role contributes significantly to improving patient outcomes and enhancing the overall quality of home health services.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the state of practice.
  • Graduation from an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  • Basic Life Support (BLS) certification.
  • Minimum of one year of clinical nursing experience, preferably in home health, community health, or related settings.
  • Strong knowledge of nursing principles, patient care standards, and home health regulations.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) degree.
  • Certification in Home Health Nursing (e.g., Certified Home Health Nurse - CHHN).
  • Experience with electronic health records (EHR) and home health documentation systems.
  • Advanced certifications such as ACLS (Advanced Cardiovascular Life Support) or wound care certification.
  • Demonstrated experience in patient education and chronic disease management.

Responsibilities:

  • Conduct thorough patient assessments to evaluate health status and identify care needs in the home setting.
  • Develop, implement, and regularly update individualized nursing care plans based on patient assessments and physician orders.
  • Administer medications, treatments, and therapies as prescribed, while monitoring patient responses and documenting progress.
  • Educate patients and their families on disease management, medication adherence, and health maintenance strategies.
  • Coordinate with physicians, therapists, social workers, and other healthcare professionals to ensure comprehensive and continuous care.
  • Maintain accurate and timely documentation of all nursing activities, patient interactions, and clinical observations.
  • Ensure compliance with all regulatory requirements, safety protocols, and organizational policies related to home health nursing.
