About the Role:
The Licensed Practical Nurse (LPN) in Home Health plays a critical role in delivering high-quality nursing care to patients within their homes, ensuring their health and comfort outside of traditional clinical settings. This position involves comprehensive patient assessments, administering medications, and collaborating closely with healthcare teams to develop and implement individualized care plans. The LPN will monitor patient progress, educate patients and their families on health management, and provide emotional support to promote overall well-being. By working in diverse home environments, the LPN ensures continuity of care and helps reduce hospital readmissions through proactive health management. Ultimately, this role is essential in enhancing patient independence and quality of life while adhering to all regulatory and safety standards.
Minimum Qualifications:
- Current and valid Licensed Practical Nurse (LPN) license in the state of Connecticut.
- Completion of an accredited practical nursing program.
- Basic Life Support (BLS) certification.
- Minimum of one year of clinical nursing experience, preferably in home health or a related healthcare setting.
- Ability to travel to patient homes within Clinton Township and surrounding areas.
Preferred Qualifications:
- Experience working in home health care or community nursing.
- Certification in wound care or chronic disease management.
- Familiarity with electronic health record (EHR) systems.
- Strong communication skills with proficiency in patient education.
- Additional certifications such as IV therapy or geriatric nursing.
Responsibilities:
- Conduct thorough patient assessments and document health status during home visits.
- Administer prescribed medications and treatments accurately and safely.
- Collaborate with registered nurses, physicians, and other healthcare professionals to develop and update individualized care plans.
- Educate patients and their families on disease management, medication adherence, and healthy lifestyle practices.
