RN/LPN Quality Assurance & Case Coordinator (On-Site)

About Us:Safe Life Home Healthcare, LLC, a 4-star patient satisfaction organization that has been proudly serving the community since 2010, is seeking a dedicated Licensed Practical Nurse (LPN) or Registered Nurse (RN) to serve as our Clinical Quality Assurance Coordinator. This role is critical in maintaining our high standards of clinical care and ensuring compliance with healthcare regulations.
Position OverviewResponsibilities:Case Coordination: Facilitate coordination between patients, care providers, and healthcare partners to ensure smooth delivery of services and continuity of care.
Quality Monitoring: Conduct regular audits of clinical documentation and patient care to identify areas for improvement.
Compliance Oversight: Ensure all clinical practices meet state and federal regulations as well as company policies.
Staff Education: Provide guidance and training to clinical staff on quality improvement initiatives and best practices.
Performance Improvement: Develop and implement quality improvement projects to enhance patient outcomes and satisfaction.
Documentation Review: Review clinical documentation for accuracy, completeness, and compliance with healthcare standards.
Experience: Minimum of 2 years of experience in home health care required.
Licensure: Current LPN/RN license in the state of Illinois.
EMR Knowledge: Experience with electronic medical record systems, preferably WellSky Kinnser EMR.
Skills: Strong analytical, organizational, and communication abilities. Attention to detail and quality-focused mindset.
Knowledge: Thorough understanding of home health regulations, clinical standards, and quality assurance processes.
