Key Responsibilities
Client Management & Coordination
- Serve as a main point of contact for clients and families
- Handle client intake calls, onboarding, and ongoing follow-ups
- Ensure high client satisfaction and address concerns professionally
- Coordinate with caregivers and internal teams to support care delivery
- Maintain accurate client documentation and records
- Assist the supervisor with referral outreach and relationship management
- Communicate with hospitals, clinics, physicians’ offices, and community partners
- Help track referrals, leads, and conversion outcomes
- Participate in community events, networking, and outreach initiatives
- Support strategies to increase client volume and retention
- Assist with scheduling, reporting, and internal coordination
- Support operational workflows and process improvement
- Provide regular updates to management on client and referral activity
- High school diploma required; college degree preferred
- Experience in home care, healthcare, client services, customer service, or sales support preferred
- Strong communication, follow-up, and relationship-building skills
- Organized, detail-oriented, and able to multitask
- Comfortable using office software and learning care management systems
- Professional, dependable, and client-focused
- Base salary: $40,000 - $50,000 annually (DOE)
