Job Overview
We are seeking compassionate and dedicated Home Health Aides (HHA), Caregivers, or DSPs to provide exceptional in-home and community-based care to individuals with diverse needs. This role involves supporting patients with daily activities, medical needs, and emotional well-being, ensuring they maintain independence and quality of life. The ideal candidate will have experience working with seniors, individuals with developmental disabilities, or those requiring specialized care such as Alzheimer's or dementia care. Responsibilities
- Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting
- Monitor patient health by observing vital signs and reporting changes to healthcare professionals
- Administer medications and manage medical documentation in compliance with HIPAA regulations
- Support patients with mobility using equipment such as Hoyer lifts and assistive devices
- Provide companionship and emotional support to promote mental well-being
- Assist with meal preparation, feeding, and nutritional needs
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment
- Document patient care activities accurately using EMR/EHR systems
- Support clients with specialized needs such as catheter care, hospice care, or Alzheimer’s/dementia-specific routines
- Follow care plans tailored to individual needs while respecting patient rights and confidentiality
Qualifications
- Prior experience in home health, assisted living, nursing homes, or long-term care preferred
- Knowledge of medical terminology and procedures such as vital signs monitoring and medication administration
- Certification in CPR & First Aid required; additional certifications such as HIPAA training or dementia care are a plus, Zero Tolerance (Abuse, Neglect & Exploitation, Infection Control, Clint Rights, Behavior Management, Safety & Emergency Procedures
- Ability to work independently with minimal supervision while maintaining high standards of patient care

- Strong communication skills for effective interaction with clients, families, and healthcare teams
- Compassionate attitude with a genuine desire to improve patients’ quality of life
- Experience working with people with developmental disabilities or memory impairments is highly valued
- Ability to pass background checks and comply with all health and safety regulations
- High School Diploma or GED
