YHDP/HMIS Data Entry Case Manager--STREET Outreach Jesup

Job Summary
Under the supervision of the Program Director, the YHDP Jesup/HMIS Case Manager assists with the daily operations of the Street Outreach Program. This position works with youth ages 17–24, as well as individuals and families experiencing homelessness or seeking services through the Street Beat Program.

The YHDP Jesup/HMIS Case Manager is responsible for HMIS data entry, maintaining data quality, generating reports, and assisting with the Coordinated Entry System.

Key duties include conducting client assessments, verifying eligibility, coordinating services and referrals, monitoring program participation, and developing individualized housing plans focused on achieving permanent housing outcomes. The Case Manager also maintains accurate client records, attends community meetings and events, and supports outreach efforts and provide assistance to the hotel/motel program.

Using a Positive Family and Youth Development approach, the YHDP/HMIS Case Manager promotes access to services for families and youth while empowering participants to achieve self-sufficiency and accomplish program goals.

Responsibilities

• · Conduct client intakes, assessments, and exits in HMIS, including coordinated entry participation
• · Ensure accurate, timely data entry, data quality, compliance, and report generation
• · Maintain complete, accurate, and confidential client files in accordance with program policies
• · Coordinate services and referrals to community partners and benefit programs
• · Develop individualized housing plans and provide housing search support and case management
• · Deliver direct client services, including life skills, independent living support, and follow-up/aftercare
• · Support youth engagement, goal planning, and connections to education, employment, and supportive services
• · Attend treatment team meetings, community meetings, and required training (minimum 10 hours annually)
• · Provide oversight of office operations and workflow, including supply coordination
• · Communicate effectively with staff, partners, and supervisors to support program outcomes
• · Assist with placement of individuals and families into the rural hotel/motel program.
• · Perform other duties as assigned.

Qualifications

• · Bachelor’s degree is accredited to college or university in social work with direct service experience, case management, and community outreach.

This Position Reports to: Street Outreach Program Director

Salary: $40,000 to $45,000

Pay: $40,000.00 - $45,000.00 per year
