Job Summary
Safe Hands Home Health Care & Hospice is seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our home health and hospice team. In this role, you'll work directly with patients in their homes, helping them regain or maintain essential communication and swallowing abilities. Your work will make a lasting impact on the quality of life for patients and their families.
This is a part-time, per-visit position with flexible scheduling and competitive rates that can be negotiated based on your experience and travel coverage area.

Key Responsibilities
- Conduct evaluations to assess speech, language, cognitive-communication, and swallowing disorders
- Develop and implement individualized treatment plans tailored to patient needs
- Provide therapy sessions in patients' homes to improve communication and swallowing function
- Educate patients and caregivers on communication techniques, swallowing safety, and exercise routines
- Collaborate with physicians, nurses, and other rehabilitation professionals to ensure coordinated care
- Document all assessments, care plans, and patient progress accurately in EMR
- Travel to patient homes across assigned coverage areas
Qualifications
- Master's Degree in Speech-Language Pathology from an accredited institution
- Active SLP license in the state of Michigan
- Minimum 1 year of experience in home health, hospice, or a related clinical setting preferred
- Strong communication, documentation, and organizational skills
- Reliable transportation and a valid driver's license
