Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team in Genesee County. The RN will provide skilled nursing care, coordinate treatment plans, and educate patients and families on managing health conditions safely at home. Responsibilities
Conduct comprehensive Start of Care (SOC), Revisit, and Discharge assessments in patients’ homes
Develop individualized care plans in collaboration with physicians and interdisciplinary teams
Administer medications, perform wound care, and monitor vital signs
Provide patient and caregiver education for condition management and recovery
Document all visits, interventions, and progress in EMR promptly and accurately
Coordinate with therapists, aides, and case managers to ensure continuity of care
Maintain compliance with all state, federal, and agency policies
Qualifications
Active and unrestricted Registered Nurse (RN) license in the state of Michigan
Minimum 1 year of recent experience in Home Health preferred
Strong clinical assessment, time management, and communication skills
Reliable transportation and valid driver’s license required
Compassionate, patient-centered approach to care
Familiarity with EMR systems (Axxess or Home Health Notify preferred)
Background Screening
