Duties
- Assist clients with activities of daily living (ADLs), such as bathing, dressing, grooming, and toileting, ensuring their dignity and comfort are maintained at all times
- Monitor patient health by observing vital signs, administering medications, and documenting medical information accurately using Electronic Health Record (EHR) systems
- Support patients with mobility using transfer methods such as Hoyer lifts and ensure safe patient transfers to prevent falls or injuries
- Provide specialized dementia and Alzheimer's care, including memory care strategies and creating a safe environment for clients with cognitive impairments
- Assist with medical procedures like catheter care, wound care, and basic life support (BLS), including CPR when necessary
- Prepare nutritious meals aligned with dietary restrictions and assist with feeding as needed while maintaining food safety standards
- Maintain strict confidentiality of patient records in compliance with HIPAA regulations and home healthcare laws
Experience
- Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred
- Demonstrated experience caring for geriatric patients or individuals with disabilities, including dementia or Alzheimer's disease
- Knowledge of medical terminology, medication administration protocols, and vital signs monitoring
- Hands-on experience with home health care assistance, hospice care, or long-term care environments
- Familiarity with EMR/EHR systems for documentation and client communication
- Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts
- Strong understanding of home & community care regulations and best practices for patient safety and confidentiality
