S & V Family Home Health Agency is seeking a compassionate, dependable, and experienced Home Health Aide (HHA) to provide in-home personal care for an adult patient located in Marathon, FL (33050).
The patient is returning home following hospitalization and requires assistance with Activities of Daily Living (ADLs), personal care, mobility, and fall prevention. The patient currently has decreased strength, endurance, and balance and requires assistance to safely perform daily activities.
This position is ideal for an HHA who is comfortable caring for an adult patient with mobility limitations and increased fall risk while promoting as much independence as safely possible.
Responsibilities
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and personal hygiene.
- Provide safe assistance with transfers, ambulation, and mobility as directed by the plan of care.
- Assist the patient with the use of prescribed mobility devices, such as a walker or cane.
- Maintain fall-prevention and safety precautions throughout the visit.
- Provide assistance with meal preparation and feeding as needed.
- Perform light housekeeping related to the patient's care and immediate living environment.
- Assist with positioning and comfort measures.
- Observe the patient's general condition and promptly report any changes or concerns to the supervising nurse/agency.
- Provide companionship, encouragement, and emotional support.
- Follow the established Home Health Aide Plan of Care and instructions provided by the supervising RN.
- Maintain patient privacy and confidentiality in accordance with HIPAA requirements.
- Complete required visit documentation accurately and on time.
- Follow infection-control and agency safety policies.
Requirements
- Valid Home Health Aide (HHA) certificate meeting Florida home health requirements.
- Previous home health or adult/geriatric caregiving experience preferred.
- Experience assisting patients with mobility limitations and fall risk is highly desirable.
- Ability to safely assist with transfers, ambulation, bathing, dressing, toileting, and other ADLs.
- Compassionate, patient, dependable, and professional.
- Strong communication skills and ability to recognize and report changes in the patient's condition.
- Must be able to follow an established care plan and work under the supervision of an RN.
- Reliable transportation to the patient's home in Marathon, Florida.
- Must meet all S & V Family Home Health Agency credentialing and employment requirements.
Important
This is a Home Health Aide position for personal care and ADL assistance. The HHA will work as part of the patient's home health team alongside skilled nursing and therapy services.
The HHA is not responsible for administering medications, removing or managing the Foley catheter, performing wound care, or providing other skilled nursing procedures. Any clinical concerns should be immediately reported to the supervising nurse.
